PCA+ APD

As an approved provider of services for Florida’s Agency for People with Disability (APD), Planned Care Agency’s partnership alignment with the States’ Agency has collectively served that mission to specifically address and serve the needs of Floridians with developmental disabilities. Floridians with developmental disabilities as defined in Florida Statutes, Chapter 393.  includes individuals with:

  • Cerebral palsy
  • Autism
  • Cerebral palsy
  • Spina bifida
  • Down syndrome
  • Prader-Willi syndrome
  • Phelan McDermid syndrome

Children aged 3-5 who are at a high risk of a developmental disability.  In this role, Planned Care Agency provides resources in our services to support people who have developmental disabilities and their families in living, learning, and working in their communities.

Planned Care Agency currently offers rehabilitative housing, skilled nursing, private and residential nursing and respite and skilled nursing.  Rehabilitative housing serves as one of our most prided and greatest challenge of services offered.  With this service, it is our obligation that housing provided represents a “home”. Though disabled, individuals must be made to feel that the care and love that goes into these locations are indicative of their personal domain.  To this end, we work in accordance to the rules and regulations set by APD to ensure that properties’ location and city compliance is welcoming and safe for the disabled community at large.

As part of the requirement, Planned Care Agency has completed a Medicaid waiver application, provided its state guided policy and procedures regarding how the disabled community will be treated and cared for, and completed the Department of Children and Family’s background check for its staff.
