Valid copy of Social Security Card

Green card/Work Permit (if applicable)/Naturalized Citizenship Documentation;
Valid Driver’s License;
Valid auto insurance card;
Valid current CPR card;
HHA - Certificate of Completion;
CNA - Certificate of Completion;
Proof of Level 2 Criminal Live Scan Background check;
Proof of Professional Liability Insurance;

- Bloodborne Pathogens, Infection Control
- Domestic Violence
- Medical Record Documentation and Legal Aspects Appropriate to Nursing Assistants
- Resident Rights/HIPAA
- Communication with cognitively impaired clients
- Medical Error Prevention and safety
- Assisting with Self-Medication
